**Please note that financial assistance for any service is limited to once every 12 months. All payments are made directly to the landlord, utility company, or pharmacy.

How do I qualify for eviction prevention assistance?

  1. You must be a Montgomery County resident living in one of the following zip codes: 20866, 20868, 20903, 20904, 20905.

  2. Must be eligible to get a referral from The Department of Health and Human Services (DHHS) or any emergency assistance coalition member (EAC).

  3. Must be able to provide:

    • Identification i.e. passport, driver’s license, ID card.

    • Proof of residency i.e. identification or utility bill in name of the applicant with the current address shown.

    • proof of income i.e. pay stubs, child and/or alimony income, workers compensation, unemployment compensation, pensions, social security income/social security disability income (SSI/SSDI).

    • contact information ie. telephone number and email address

  4. Copy of rent or mortgage statement. Rent or mortgage payment must be past due and/or and eviction notice.

How do I qualify for utility disconnection prevention assistance?

  1. You must be residing in the following zip codes in Montgomery County: 20866, 20868, 20903, 20904, 20905.

  2. You must be able to provide:

    • past due utility bill in the name of the applicant with the current address shown.

    • proof of income i.e. pay stubs, child and/or alimony income, workers compensation, unemployment compensation, pensions, social security income/social security disability income (SSI/SSDI).

    • contact information ie telephone number and email address

How do I qualify for prescription medication assistance?

  1. You must be residing in the following zip codes in Montgomery County: 20866, 20868, 20903, 20904, 20905.

  2. You must be able to provide:

    • Proof of residency i.e. driver’s license or utility bill in the name of the applicant with the current address shown.

    • Social security number

    • Proof of income i.e. pay stubs, child and/or alimony income, workers compensation, unemployment compensation, pensions, social security income/social security disability income (SSI/SSDI).

    • A current prescription (s).

    • Contact information i.e. telephone number and email address

  3. A confidentiality form is required.
